Carruth Data Breach

Carruth Compliance Consulting (Carruth) is the third-party administrator that handles 403(b) and 457(b) retirement savings plan for many Oregon school districts, including Reynolds School District. Carruth discovered suspicious activity on their computer systems. An investigation revealed that unauthorized access to Carruth’s network occurred on December 21, 2024, resulting in the compromise of sensitive employee data for Carruth’s clients, including RSD. Carruth’s customers include many Oregon school districts, ESDs and other organizations.

Carruth reported that upon learning of the Incident, they began working with third-party specialists to investigate the activity, and then notified the Federal Bureau of Investigation. Carruth also engaged a sub-contractor to handle processing of information coming in from its clients. For the foreseeable future, no further retirement account transactions from Reynolds School District employees will be processed by Carruth.

FAQ

Was my information affected?

It appears that the Carruth Incident may potentially affect all individuals who were employed by Reynolds SD between December 2008 and January 2025. We encourage all potentially affected current and former employees to take the steps outlined below to monitor and protect their personal information.

 

What information was affected?

Carruth reported that the affected information may include employee and beneficiary Form W-2 Wage and Tax Statement information (names, Social Security numbers, mailing addresses and compensation information), dates of birth, financial account information, email addresses, driver’s license numbers and medical billing information (but not medical records).

 

Was Reynolds SD’s information system affected?

No. The Carruth Incident is a third party incident involving the Carruth systems – it has nothing to do with Reynolds SD’s systems. Reynolds SD’s systems were not involved in the Carruth Incident and Reynolds SD’s systems remain secure.

 

Is my retirement account safe?

There is no evidence that retirement accounts were affected, and we are in communication with the custodians of those accounts to ensure they remain secure.

 

What if I do not/did not participate in Reynolds SD’s 403(b) and 457(b) plans administered by Carruth? Why would my data be affected?

While Carruth provided third party administrative services for Reynolds SD’s 403(b) and 457(b) retirement savings plan, Carruth also monitored our contributions compliance.  This required us to provide data for all employees to ensure the District's practices were in compliance with IRS limits. 

 

What is Reynolds SD doing?

As soon as we learned about the incident, we began working with Carruth and multiple other parties to understand the full scope of the Incident, to ensure all affected employees will be directly notified and provided appropriate remediation services, and to ensure Carruth is taking appropriate steps to mitigate the impact on our employees. We have mailed notification letters to all affected individuals notifying them about the Incident and offering the opportunity to enroll in credit monitoring and identity theft protection services through IDX.

 

Was my information compromised/misused?

Receiving a letter does not mean that you are, or will be, a victim of identity theft. At this time, Carruth has not provided any evidence that any impacted information has been or is likely to be misused. However, we encourage you to follow the recommendations provided in the letter you received.

 

Is there anything I need to do given the potential exposure of my information?

Although there is no evidence that any information has been or will be misused, we recommend that individuals review the letter that they received regarding the incident and follow the steps outlined to further protect their information.

 

Who is IDX?

IDX is a leading data incident response services provider that helps protect people who may be affected by data security incidents. Carruth retained IDX to provide complimentary credit monitoring and identity protection services to our employees and answer questions you may have about the incident.

 

I am having trouble enrolling with IDX. Is someone from Reynolds SD able to assist me with enrolling in the services provided by IDX?

Unfortunately, each impacted individual must enroll separately in the services provided by IDX. In order to enroll in the services provided by IDX, you must contact IDX at 1-877-390-8299 and provide them with information they request. 

 

What should I do?

  • Enroll in credit monitoring and identity restoration services. Carruth is offering free credit monitoring and identity restoration services through IDX, a firm that provides identity protection services to consumers affected by data security incidents. To enroll, please call IDX at 1-877-390-8299.
  • Monitor your accounts. Regularly review your bank accounts, credit card statements, retirement accounts and other financial accounts for any suspicious activity. If you see anything unusual, report it to your financial services provider immediately.
  • Check your credit reports. You are entitled to one free credit report annually from each of the three major credit reporting bureaus (Equifax, Experian and TransUnion). Visit www.annualcreditreport.com or call 877.322.8228 to order your free reports. 
  • Consider placing a fraud alert on your credit file. Fraud alerts notify creditors to verify your identity before issuing new credit. You can place an initial fraud alert (lasting one year), or an extended fraud alert (lasting seven years), at no cost, if you believe you are a victim of identity theft. 
  • Consider placing a Credit freeze on your credit file. Credit freezes prevent credit bureaus from releasing your credit report without your explicit consent. This makes it harder for identity thieves to open accounts in your name. You can place a credit freeze on your credit file at no cost.
  • Report any suspicious activity. If you suspect you are a victim of identity theft, you should file a police report.  You can also report it to the Federal Trade Commission at www.identitytheft.gov or 877.ID.THEFT (877.438.4338).

Tax Form 1095-C

2024 1095-C, Employer Provided Health Insurance Offer & Coverage

Under the Employer Reporting and Improvement Act and the Paperwork Burden Reduction Act, Applicable Large Employers (ALEs), such as Reynolds School District, have a new option to meet their furnishing obligations that no longer requires them to automatically furnish Form 1095-C to full-time employees. Instead, employers can opt to provide these forms only upon employee request.

With this new legislation, Reynolds School District has opted to do the following:

  • Post all 1095-C forms to iVisions Employee Access for employees to download at their convenience.
  • Upon receiving a request for a 1095-C form, Reynolds will furnish the employees Form 1095-C within 30 days after the date of the request.

What is a 1095-C?

1095-C is one of the two statements you will receive to fulfill the reporting requirements of the Affordable Care Act (ACA).  Employers are to provide statements (1095-C) to full-time employees as defined in the law. The second statement (1095-B) will be provided by your health plan carrier (Kaiser or Moda).

Form 1095-C is an important tax document which you may need when completing your 2024 federal income tax return. Please keep this 1095-C statement with your Form W-2 and other wage and tax information documents. You will answer a question about your 2024 medical coverage when you file your federal income tax return (Most will simply need to check a box indicating that they had medical coverage for the entire year).

  • Part II:
    • Box #14:  States whether coverage was offered to you from Reynolds School District.
    • Box #15:  States your share of the lowest cost monthly premium available to you for self-only coverage, not necessarily the premium for the coverage in which you are enrolled.
    • Box #16:  States the Safe Harbor used, if applicable.
  • Part III:  This section on the 1095-C statement is purposefully blank. The health plan carrier (Kaiser or Moda) will report actual enrollees, including dependents, on Form 1095-B.

Oregon PERS

PERS stands for Public Employees Retirement System. This is a state operated pension program for public employees which both Employees and their Employers contribute to. After a six (6) month waiting period, a deduction of 6% of your gross wages will be contributed from your paycheck to your PERS account. Reynolds matches this percentage at 5.59% which funds retirement benefits, death benefits, and administrative expenses. These percentages are determined and updated by the PERS Board every even numbered year going in to effect every odd numbered year. Retirement Information

Continuing Health Insurance

If you are retiring under the age of 65, you may remain on the OEBB benefits if you choose. However, you may be liable for the full cost of the insurance plans. Please reach out to the Benefits Team to determine the cost of continuing your OEBB benefits during retirement.

Payment for continuing health insurance will need to be arranged via an ACH pull from a designated checking account. Please include a voided check with the ACH Payment Authorization Form. ACH deductions will be on or about the 10th day of each month. If the 10th falls on a weekend or a holiday, the payment will be deducted on the following business day.

All supplemental insurances would end with your employment with Reynolds. Some policies are portable, meaning you can contact the provider and they can direct bill you for the policy.

 

COBRA Information

COBRA is guaranteed continuation of your previous OEBB coverage, administered by BenefitHelp Solutions (BHS). Regardless of your health status, COBRA law allows eligible parties losing OEBB coverage to continue that same coverage for at least 18 months.​

COBRA premiums are equal to the full OEBB premium plus two percent. Keep in mind, Reynold's contributes toward your premium when you were eligible for OEBB coverage, so the amount you were paying was not the full premium amount. OEBB publishes full premium rates and COBRA rates on the OEBB website.​

When OEBB coverage ends, BenefitHelp Solutions mails COBRA information to the eligible party, including rates and enrollment instructions/forms. Anyone enrolling in COBRA coverage will work directly with BenefitHelp Solutions, making payments directly to them.

When Reynolds enters a termination date for your coverage in MyOEBB, the online benefit enrollment system, that data gets transferred to BenefitHelp Solutions within a week and they will mail your personalized COBRA packet/application to your mailing address that is listed in MyOEBB.

OEBB has contracted with BenefitHelp Solutions (BHS) to administer COBRA. Payments must be made directly to BHS. You may contact BHS directly at (800) 556-2230 or online at: www.benefithelpsolutions.com/oebb.

COBRA coverage is for the same OEBB plan(s) you had as an active employee, so you will not have the option to switch plans. You may choose whether or not to continue medical or dental or vision coverage.​

COBRA participants may make coverage changes if they experience a Qualified Status Change (QSC) or during Open Enrollment. Available options and the associated premium costs for OEBB COBRA participants are distributed by BenefitHelp Solutions each August.  Questions regarding COBRA Open Enrollment should be directed to BenefitHelp Solutions at (800) 556-2230.

Although COBRA coverage changes can be made at certain times, plan changes are not allowed within a certain type of coverage. For example, you cannot change from one medical plan to another medical plan.
