RSD Work Share: Initial Claim Form Process & Instructions
Filing the Initial Claim Form - THE ONLY claim you will file for the RSD Work Share Program
Complete the Work Share Initial Claim Form and send signed copy to WorkShare_HCM@rsd7.net. RSD will submit all forms directly to the Oregon Employment Department through a secure file transfer.
To avoid delays, the Initial Claim Form must be completed and submitted to WorkShare_HCM@rsd7.net or dropped off by Thursday, June 4, 2020 at 5:00 pm. Employees who become eligible prior to July 31 should complete the Initial Claim Form during this time.
Note: If you submitted an Unemployment claim in the past two months, regardless if it was related to RSD or another employer, you do not need to complete this process.

If you do not have access to a device or the internet, in-person support will be available by appointment only at the Transportation Department located at 20311 NE Glisan St, Building C, Fairview, OR 97024 from Monday, June 1 – Thursday, June 4 from 9:00 am to 2:00 pm. In order to maintain social distancing, we ask that you follow these guidelines if you need to come in for support:
- Make an appointment using this form.
- If possible, come alone, or limit the number of individuals who must come with you. (If you would like to bring someone as an interpreter, please do so.)
- Wear disposable or reusable (cloth) face coverings
- Wear gloves
- Follow all directions of the volunteers
- Maintain proper social distancing at all times

Tips prior to starting your application:
Be prepared with the following information:
- DOWNLOAD the form to your computer or device. Do not try to fill out in a web browser.
- Access to your iVisions Web Portal (username and password)
- If you need assistance with your iVisions password, please submit an IT Ticket: https://sos.rsd7.net/portal
- Social Security Number
- Your most recent start date with RSD. If your employment with RSD started before May 1, 2018, an estimated date will be adequate. If you started after May 1, 2018 please email HCM at Reynolds_HR@rsd7.net to get your start date.
- If you have worked for RSD for less than 2 years (as of June 1, 2020) or have worked for other employers in the last 2 year, you will need work history for other employers for the last 2 years, including dates of employment, your employers' business names, addresses and phone numbers, gross earnings and rate of pay.
- If you are not a citizen of the United States, you will need your Alien Registration Number and documentation.
- If you are a veteran, you will need your DD-214.
- Gross pay - entering an estimate will be fine.
You will be asked about other employment. You must answer this honestly - even if your work with the other employer has not changed.
Instructions for completing the form:
In the top section of page 1:
- Social Security Number: enter the full number. Your application will be submitted in a secure format to the Oregon Employment Department.
- Name (Last, First, MI): enter your legal full name. This will ensure records align with your social security card as well as your RSD personnel record. Do not enter a preferred name, even if you use it professionally at RSD.
- Phone: enter the primary phone number.
- Mailing address: this will be the address the initial check will be mailed.
- Ethnicity: mark all applicable.
- Date of Birth: enter in full format: mm/dd/yyyy
- Work Share Employer: Reynolds School District, Multnomah #7
- Phone: 503-661-7200
- Address: 1204 NE 201st Ave, Fairview, OR 97024
- Employment start date: enter your most recent start date with RSD. If your employment with RSD started before May 1, 2018, an estimated date will be sufficient.
- Job Title: enter your current job title with RSD
For items A – H: enter yes or no as applicable for the last 18 months. If mark yes for any item, complete the additional question(s).
- For item A: mark “no” for RSD. Mark “yes” if you worked for an agency of the Federal Government.
- For item F: mark "yes" if you are a U.S. citizen and leave other two questions in section F blank.
- If you are not a U.S. citizen, mark "no" and then answer the next questions if you are legally able to work in this country. If you answer "yes" please provide your Alien Registration Number in the box provided.
Do not enter any data in the “For Office Use Only” section.
On page 2:
- Enter information for other employers in this section only if RSD has not been your only employer in the past 2 years (from June 1, 2018) or you have been employed by RSD for less than two years (start date after June 1, 2018). If you have worked for RSD for at least two years and RSD was your only employer, you do not need to complete this section.
- Read through the certification section. Check box.
- Signature: sign electronically or print and sign.
- If you are having trouble with the signature box, you can choose the pen tool in Acrobat and choose the blue pen to sign.
- When you open, click on the pen in the toolbar:

- Date: enter date you completed the application.

Reminder: You only need to complete this claim one time. RSD is participating in the Work Share program so you will not need to submit a weekly claim form through OED. Some eligible employees who earn additional income from other employers or receive retirement pay will have to submit information on a weekly basis to RSD to report through the Work Share program. How will I receive payments through the Work Share Program and CARES Act?
Your first payment will be a paper check that is mailed to the address you provide on the initial claim form. After your first payment, you will receive payments that are preloaded onto a debit card (Visa ReliaCard) that is mailed to you.
Alternatively, you can elect to receive these payments via direct deposit. You will not be able to set up Direct Deposit until after your claim has been approved. If you would like to set up Direct Deposit, complete this authorization form (English | Spanish) and submit it to WorkShare_HCM@rsd7.net.
