Tax Form 1095-C

2025 1095-C, Employer Provided Health Insurance Offer & Coverage

Under the Employer Reporting and Improvement Act and the Paperwork Burden Reduction Act, Applicable Large Employers (ALEs), such as Reynolds School District, have a new option to meet their furnishing obligations that no longer requires them to automatically furnish Form 1095-C to full-time employees. Instead, employers can opt to provide these forms only upon employee request.

With this new legislation, Reynolds School District has opted to do the following:

  • Post all 1095-C forms to iVisions Employee Access for employees to download at their convenience.
  • Upon receiving a request for a 1095-C form, Reynolds will furnish the employees Form 1095-C within 30 days after the date of the request.

What is a 1095-C?

1095-C is one of the two statements you will receive to fulfill the reporting requirements of the Affordable Care Act (ACA).  Employers are to provide statements (1095-C) to full-time employees as defined in the law. The second statement (1095-B) will be provided by your health plan carrier (Kaiser or Moda).

Form 1095-C is an important tax document which you may need when completing your 2025 federal income tax return. Please keep this 1095-C statement with your Form W-2 and other wage and tax information documents. You will answer a question about your 2025 medical coverage when you file your federal income tax return (Most will simply need to check a box indicating that they had medical coverage for the entire year).

  • Part II:
    • Box #14:  States whether coverage was offered to you from Reynolds School District.
    • Box #15:  States your share of the lowest cost monthly premium available to you for self-only coverage, not necessarily the premium for the coverage in which you are enrolled.
    • Box #16:  States the Safe Harbor used, if applicable.
  • Part III:  This section on the 1095-C statement is purposefully blank. The health plan carrier (Kaiser or Moda) will report actual enrollees, including dependents, on Form 1095-B.

paper enrollment form is required to enroll in benefits. Please complete this form and submit it to benefits@rsd7.net within 30 days of your hire date.

Oregon PERS

PERS stands for Public Employees Retirement System. This is a state operated pension program for public employees which both Employees and their Employers contribute to. After a six (6) month waiting period, a deduction of 6% of your gross wages will be contributed from your paycheck to your PERS account. Reynolds matches this percentage at 13.84% which funds retirement benefits, death benefits, and administrative expenses. These percentages are determined and updated by the PERS Board every even numbered year going in to effect every odd numbered year. 403(b) Tax-Sheltered Annuity (TSA)

Employees interested in contributing more to their retirement can do so voluntarily to a 403(b) TSA plan or a 457(b) deferred compensation plan. Choose from our approved vendor list located on our staff portal benefits page under Retirement Information. To enroll, select and create a plan with one of our listed vendors. Once the account is created, complete a salary reduction agreement and return it to benefits@rsd7.net. Confirm with your advisor if they will complete this on your behalf.

Retirement Information

Continuing Health Insurance

If you are retiring under the age of 65, you may remain on the OEBB benefits if you choose. However, you may be liable for the full cost of the insurance plans. Please reach out to the Benefits Team to determine the cost of continuing your OEBB benefits during retirement.

Payment for continuing health insurance will need to be arranged via an ACH pull from a designated checking account. Please include a voided check with the ACH Payment Authorization Form. ACH deductions will be on or about the 10th day of each month. If the 10th falls on a weekend or a holiday, the payment will be deducted on the following business day.

All supplemental insurances would end with your employment with Reynolds. Some policies are portable, meaning you can contact the provider and they can direct bill you for the policy.

 

COBRA Information

COBRA is guaranteed continuation of your previous OEBB coverage, administered by BenefitHelp Solutions (BHS). Regardless of your health status, COBRA law allows eligible parties losing OEBB coverage to continue that same coverage for at least 18 months.​

COBRA premiums are equal to the full OEBB premium plus two percent. Keep in mind, Reynold's contributes toward your premium when you were eligible for OEBB coverage, so the amount you were paying was not the full premium amount. OEBB publishes full premium rates and COBRA rates on the OEBB website.​

When OEBB coverage ends, BenefitHelp Solutions mails COBRA information to the eligible party, including rates and enrollment instructions/forms. Anyone enrolling in COBRA coverage will work directly with BenefitHelp Solutions, making payments directly to them.

When Reynolds enters a termination date for your coverage in MyOEBB, the online benefit enrollment system, that data gets transferred to BenefitHelp Solutions within a week and they will mail your personalized COBRA packet/application to your mailing address that is listed in MyOEBB.

OEBB has contracted with BenefitHelp Solutions (BHS) to administer COBRA. Payments must be made directly to BHS. You may contact BHS directly at (800) 556-2230 or online at: www.benefithelpsolutions.com/oebb.

COBRA coverage is for the same OEBB plan(s) you had as an active employee, so you will not have the option to switch plans. You may choose whether or not to continue medical or dental or vision coverage.​

COBRA participants may make coverage changes if they experience a Qualified Status Change (QSC) or during Open Enrollment. Available options and the associated premium costs for OEBB COBRA participants are distributed by BenefitHelp Solutions each August.  Questions regarding COBRA Open Enrollment should be directed to BenefitHelp Solutions at (800) 556-2230.

Although COBRA coverage changes can be made at certain times, plan changes are not allowed within a certain type of coverage. For example, you cannot change from one medical plan to another medical plan.
