Parent Opt-Out and Exemption Guidelines for State Testing

Parent Request: Opt-out of Oregon English Language Arts and/or Mathematics Summative Assessments (2023-2024 Parent Request Forms are posted at the bottom of the screen)

Oregon state law requires all districts to inform parents that a form is available from the Oregon Department of Education that provides the opportunity to opt your child out of the ELA and Math assessments. The form must be filed annually and is available in multiple languages.

  1. Parents must return completed form to the school office. This is an annual requirement.
  2. Incomplete or modified forms will not be accepted.
  3. Parents may not write in additional assessments on the state opt-out form.
  4. Forms will be accepted at any time. If a form is submitted after a student has begun testing, the student will stop testing but the items answered up to that point will be scored. The student will receive a performance level and/or overall score based on the incomplete submission.

The purpose of the assessments is to show individual student growth over time and to provide supplementary student-specific data that teachers can use to ensure each and every child is on track to reach goals for college and career readiness.

Parent Exemptions for Kindergarten, Science, ELPA Summative, Alt-ELPA and ELPA Screener, and Extended Assessments (2023-2024 Parent Exemption Form will be posted soon)

Under Oregon Administrative Rule 581-022-1910, a "school may excuse students from a state required program or learning activity, where necessary, to accommodate students' disabilities or religious beliefs." To comply with state requirements, an application must be completed in its entirety that includes a) the reason for the request and b) the proposed alternative for an individualized learning activity which meets the goals of the learning activity being exempted. The application applies to the following state assessments:

  • Science
  • ELPA Summative, Alt-ELPA and  ELPA Screener
  • Extended Assessments in ELA, Math, and Science
  • Kindergarten

Please fill out the form found on this page and return it to your child's principal. The principal is asked to take a meeting with you to review the application and discuss the available accommodations for the requested content exemptions. The application is then forwarded to Rachel Aazzerah at 1204 NE 201st Avenue, Fairview, OR 97024, for final review and approval.
