All Oregonians age 5 and older are now eligible to get vaccinated against COVID-19, and children as young as 12 authorized to get a Pfizer booster! Visit the OHA COVID-19 vaccine webpage for more information.

For ages 5-14: Children ages 5-14 must be signed up for an appointment by a parent/guardian (if making an appointment instead of coming to a drive-up or walk-in site). If someone other than a parent or guardian accompanies the 5- to 14-year-old, they will need to provide proof of parental or guardian consent. Proof of consent is either:

  • A signed consent form (English / Español)
  • Parental or guardian consent is required to vaccinate people under 14 years old, but the parental or guardian consent requirement does not necessarily mean a parent or guardian must go with the youth to receive the vaccination. Written consent may be obtained in advance. You should contact the vaccine provider or vaccination clinic to determine whether a parent or guardian must be present.

For ages 15-17: Under Oregon law, those who are at least 15 do not need a parent/guardian to consent to medical care. They may make an appointment (or choose a drive-up or walk-in site) and come to a vaccination alone.

Reynolds Student Health Center provides weekly COVID-19 Vaccine Clinics for youth ages 5–19 and their families on Mondays from 2:15pm to 3:30pm and on Fridays from 8:00am to 10:30am. Pfizer boosters for youths 5 to 19 are also available. Vaccination is free and available to all Multnomah County youth ages 5–19, regardless of immigration or insurance status.

Call 503-988-5558 to schedule. By appointment only; no drops-ins. Youth ages 15–14 must be accompanied by a parent or guardian or have a signed consent form. COVID-19 testing is also available for youth ages 5 to 19 only and only by appointment. No adults and no drop-ins, please.
 

Multnomah County Clinics

Multnomah County hosts regularly scheduled vaccine clinics Tuesday and Thursday 9am - 4pm, Friday 11am - 6pm.
